who want to work with our private duty (home health care) patients in Maryland.
Requirements

Current, unencumbered Maryland RN or
LPN license |
| 2. |
A minimum of 1 year clinical
experience (including pediatric direct patient care |

within the last 2 years) |

Required
We'll verify your Maryland RN or LPN license online, but during your
face-to-face interview, you will be required to submit the following
documents:

A Statement of Clinical Experience
including Pediatric Direct Patient Care |
| 2. |
A copy of the front and back of your
CPR card |
| 3. |
A list of at least, 2 job references
and one personal reference |
| 4. |
A copy of your driver's license or
government-issued photo ID card |
| 5. |
A copy of your employment
eligibility document (if you are not t a US citizen or |
| |
a permanent resident, i.e., if you
don't have a "green card") |
| 6. |
A copy of your Social Security Card |
| 7. |
PPD or Chest X-ray |
| 8. |
Hepatitis B Vaccination (if
available, or you can submit it later) |
