Referrals

We receive and accept service referrals from the following:

■	Individuals
■	Case workers
■	Family members
■	Community providers
■	Department of Social Services
■	Other service providers (e.g., doctors)
■	Other healthcare facilities (e.g., hospitals, nursing homes, etc)

We respond to all referrals within 24 hours of receipt. We will be right there with you to team up with other care providers, and to monitor progress and to adjust our services as your needs change. Indeed, we are focused on providing you with the highest level of quality care while offering the best value.
