Gastroenterologic &
Ostomy Care
(Services available to both adults and children)
Our RNs and LPNs are members of a multidisciplinary team of
care-providers and expects to coordinate with patients’ physicians and other
care-providers to support patients (including
children) who may be carrying invasive medical
devices.

Conditions/Diagnosis for Which Referrals Would be Accepted
REM patients must
meet the requirements of the
underlying diagnosis and
conditions stipulated by the REM Program. For others who are interested
in our gastroenterologic and ostomy care program, below are examples of conditions/diagnosis
that are required for acceptance:

What We Do

1. |
Assessment of the
patient’s status |

2. |
Where applicable and
upon demand, we provide pre-operative education and/or
stoma site selection and marking |
|
3. |
Providing post-operative
instruction for the patient and caregiver on ostomy care |
|
4. |
Coordinating, whenever
necessary, with treating physicians, enterostomal therapists
or other specialists, hospitals or other associated care providers
for the fitting and
management of an appropriate ostomy appliance for normal and complex
ostomies |
|
5. |
Treating peristomal skin
breakdown |
|
6. |
Coordinating with
treating physicians, dieticians or other specialists, hospitals or
other associated care givers for dietary counseling and sexual
adjustment support |
