The Maryland REM Program

As members of various multidisciplinary teams of caregivers, we work with case managers and patients under the Maryland Medicaid Rare and Expensive Case Management (REM) Programs, to provide the required intensive nursing services and specialized care to children and adults with rare, expensive, and chronic medical conditions.

For patients that meet the requirements of the underlying diagnosis and conditions stipulated by the REM Program, we provide the following skilled nursing services:

■	Pediatric Care
■	Gastroenterologic and Ostomy Care
■	Medication Management
■	Wound Care
■	Diabetic Management
■	Well Mom/Baby Care
■	Pain Management

Our services are provided by Maryland Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), in accordance with patients' physicians' orders.
